About Dr. Caylor:
Jeffrey L. Caylor, D.O. has been in practice exclusively in Foley since 1996. He is Board Certified in General Surgery and performs general, bariatric and laparoscopic surgery. He prides himself on staying up to date on the latest technology and continues to advance his education in this area.

Dr. Caylor has a true compassion for his patients and remains very active in the community. Presently he serves on the board of the American Cancer Society, the local Rotary Club and his church. He is member of the American Society of Bariatric Surgeons, Medical Association of the State of Alabama, Baldwin County Medical Society and other professional organizations.

About The Practice:
The office is located at 1668 North Pine Street in Foley, Alabama. Office hours are Monday through Friday 9:00am until 5:00pm. For more information or to schedule an appointment please call 251-970-1295. Most insurance are accepted. We are presently accepting new patients and are always happy to assist our established patients with their surgical needs.

Insured Patients:
We are a participating provider with most insurance companies. As a courtesy to our patients we will file your insurance claim on your behalf. All co-pays, deductibles and non-covered charges are to be paid in full at the time services are rendered. All fees regardless of insurance coverage are the patient’s responsibility.

We do not accept Medicaid.

Non-Insured Patients:
Patients having no insurance coverage are asked to pay 100% of the charge at the time service is rendered. In hardship or emergency situations arrangements may be made through our Patient Account Manager.

Billing and Collection Procedures:
Statements are sent out monthly to patients having outstanding balances. We request you pay the balance owed in full within 30 days of receipt of the statement. Account balances that are in default may be turned over to an outside collection agency and/or attorney. All fees incurred as a result of this are the patients’ responsibility.

Return Check Policy:
Any check that is returned to us for insufficient funds will be charged back to the patients account with an additional charge of $28.00. Patients will be contacted immediately and should bring cash or a money order to the office and pick up the bad check.

Most Frequently Asked Questions:
1. Why did you send me a bill when I have insurance?
1a. All accounts receive a bill monthly if they have an outstanding balance.

2. Can I make payment arrangements?
2a. In hardship situations we may be able to make arrangements through our Patient Account Manager. That direct line number is 251-970-1946.

3. My insurance did not pay when you filed my insurance the first time, will you refile it?
3a. That depends on the reason the claim was denied. In some cases we will refile the claim one more time.

4. What is my minimum payment?
4a. Unless prior arrangements have been made any outstanding balance should be paid in full upon receipt of the statement.

5. Did you file my insurance?
5a. If you provide us with your correct insurance information we will file your claim as a courtesy to you.

6. Why are doctor charges not included in my hospital bill?
6a. We are not affiliated with the hospital; therefore you will receive a separate bill for Dr. Caylor’s services.

7. Will you call me with my test results?
7a. Yes, upon receipt of your results and review by your doctor we will call you with the results.

8. How many days will I be off after my surgery?
8a. There are many variables here. Your doctor will discuss this with you at your appointment prior to surgery.

9. Are you a Blue Cross Blue Shield Preferred Medical Doctor?
9a. Yes
